Psychosocial Rehabilitation (PSR)

A recovery-focused, skill-building service for adults living with severe and persistent mental illness — designed to restore functioning and support meaningful community participation.

What PSR is

Psychosocial Rehabilitation is a structured, Medicaid-billable service delivered in accordance with NC DHB Clinical Coverage Policy 8A. It is designed for adults whose mental illness has significantly impaired their functioning in work, school, social, and daily-living domains — and who benefit from a structured program to rebuild those skills.

PSR is not therapy. It is not passive. It is active, goal-directed learning and practice — delivered in a group-based setting with individualized treatment plans — that helps each person acquire, restore, or relearn the skills they need to live as independently as they choose.

What PSR looks like at Empowering Minds

Our PSR program combines structured daytime programming with individualized skill coaching. Each client receives a Person-Centered Plan developed collaboratively with their treatment team.

  • Independent living skills: budgeting, meal planning, housekeeping, medication management, transportation.
  • Social & communication skills: assertiveness, conflict resolution, healthy boundaries, community engagement.
  • Vocational readiness: job search, resume building, interview skills, workplace expectations — a natural bridge to our Supportive Employment program.
  • Symptom management & coping: recognizing early warning signs, applying coping strategies, wellness planning.
  • Community integration: accessing recreation, faith communities, volunteer opportunities, and natural supports.

A typical week

PSR participants attend structured programming during the day, combining group activities with one-on-one skill coaching. Sessions follow each person's individualized plan — some weeks lean toward vocational readiness, others toward independent living or coping skills. Attendance and progress are documented in the client's electronic health record after every session.

Who qualifies for PSR

PSR is appropriate for adults (age 18+) with a qualifying diagnosis who meet medical-necessity criteria under NC Medicaid. Candidates typically have a severe and persistent mental illness and functional impairments that interfere with daily life. Eligibility is confirmed through a Comprehensive Clinical Assessment (CCA).

How to access PSR

PSR can be accessed through self-referral, family referral, or referral from a primary care provider, hospital, school, LME-MCO care coordinator, or other community partner. Our intake team will guide you through the process — including verifying Medicaid eligibility and coordinating the required clinical assessment.

How progress is measured

We document progress at every session and formally review each client's Person-Centered Plan on a regular schedule. Goals are concrete and measurable — for example, "prepare and follow a weekly grocery list within a fixed budget for four consecutive weeks" rather than a vague "improve independent living." Outcomes are reported to the client, family (with authorization), and LME-MCO as required.
